Terms and Conditions for Optional 1 or 2 Year Agreement: Customer agrees to accept the following terms and conditions for telecommunication services, which may include but not be limited to, any combination of Voice, Internet, and Long Distance (the “Service” or “Services”). This Agreement shall stay in effect for the term selected above from the date signed below. Customer must maintain this service for the length of the term selected or they will be subject to a $100 cancellation fee. Customers selecting a one year agreement will receive their first month free in the form of a credit to their bill for the bundle amount selected above. Customers that select a two year agreement will receive their 1st and 13th month free in the form of a credit to their bill for the bundle amount selected above. If a Customer has selected a one or two year agreement for bundled services and elects to upgrade to a more expensive bundle during the term, they will be credited the difference between those services in the month they elect to upgrade. If a Customer terminates a Service that was part of a pricing bundle the remaining Services will be adjusted to the normal “retail rates”, in addition to the $100 cancellation fee. At the end of the term prices for services will be adjusted to the normal “retail rates” in effect at that time, unless customer signs a new agreement for bundled services being offered at that time. Upon written notice, Southeast Nebraska Telephone Company may, without incurring any liability, temporarily discontinue Service or terminate this Agreement if Customer fails to submit timely payment upon receipt of a valid bill. Any Service may be refused or this Agreement terminated immediately if: 1) it is in violation of any applicable Federal or State tariffs, statutes or regulations; 2) information provided by Customer is false or inaccurate; or 3) the Service is used in a manner that is unethical, illegal or interferes unreasonably with the rights of other persons. This Agreement shall be governed by all applicable State and Federal regulations, statutes and tariffs. Tariffs are on file with the Nebraska Public Service Commission, and may be accessed and reviewed by the public.
